Billing Services to Insurance

Upon receipt of PCP referral for OT evaluation, we will contact you to schedule an initial evaluation and first spelling session (Approximately 60-90 minutes in total).

OT will discuss results of assessment, recommend total therapy minutes per week, and discuss scheduling weekly sessions.

Following OT evaluation, we will request an OT treatment script from the PCP. We cannot begin weekly sessions until we receive this script. It may be helpful for you to call the PCP often to check-in on the script. We will contact you once it is received to get weekly sessions scheduled.

Private Pay

Continue scheduled appointments with S2C Practitioner as recommended. Payment is due at the time of each session.

*Please note that after-school spots are limited at this time. Spelling sessions will be scheduled as often as determined by family and as your S2C Practitioner's work schedule allows.

Request an Occupational Therapy (OT) Evaluation Referral to Integrated Therapy Services in Pocahontas. We cannot schedule an initial evaluation until we receive this document.

Complete Service Agreement/Contract.

Private Pay

Upon receipt of completed Service Agreement/Contract, we will reach out to schedule an initial session (60-minutes) with your speller at Integrated Therapy Services. We will spend this session spelling (as tolerated) & discussing the process further with caregiver/parent(s).

No documentation from PCP is required.

Following the consultation, we will send you a link to the Service Agreement/Contract for you to complete. We must have this signed upon determination of beginning services.
